Approximately 36 million people in the US, experience an overnight hospital stay, every year. That represents nearly 8% of the US population. From that group, approximately 15% - 20% (5.4m - 7.2m people) experience an unplanned readmission, within 30 days of hospital discharge1.
For this project, we will build tools to help medical professionals assess unplanned readmission risk for patients at discharge time.
We will build tools using machine learning and modern information retrieval systems. This will help medical professionals collect risk signals on patient trajectories and inform interventions, improving patient outcomes.
